Dr Wood reviews the duration of signs, the dog’s condition at presentation, lactate measurements and response to initial fluids/decompression, and expected support needs. These findings inform his decision; there is no automatic score-to-price rule.
Tier 3 includes two nights and splenectomy if needed. Tier 4 includes four nights, splenectomy and partial gastrectomy if needed. Partial gastrectomy is an inclusion, not an automatic trigger for changing the price during surgery.
Evidence for referring vets
Record duration to first presentation, ambulation and mentation, initial/repeat lactate with sample times and treatment between them, and current support. Being carried is not equivalent to being unable to walk.
The post-treatment lactate cutoff of 6.4 mmol/L in the Zacher study was derived in the subgroup with initial lactate above 9 mmol/L. It is not a universal financial threshold. Do not delay treatment to obtain pricing measurements.

The operation may be similar between packages. Preoperative condition, perfusion, lactate and response to initial treatment, available organ-function/coagulation results and expected care needs guide Dr Wood’s selection. Tier 3 includes two nights; Tier 4 includes four nights.
Albumin and other blood results support the assessment, but no single result assigns a tier. Published outcome studies do not determine an individual patient’s recovery.

Urine culture is included to check for bacteria in the urine, which can accompany uterine infection. A prospective study found concurrent bacteriuria in 33% of dogs sampled. Culture results are interpreted with clinical signs; a positive culture is not by itself proof of symptomatic urinary infection.
Condition, hydration, PCV, renal values and white-cell findings help plan care before surgery. Open or closed cervix alone does not automatically select a tier.
